Job Description

Job Summary:

Primary function of the Home Health Aide is to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Duties and Responsibilities:

  • Provide health care services in patients residences
  • Perform domestic and household tasks
  • Transport and accompany patients to doctors office or to hospital
  • Administer simple prescribed medications
  • Assist with clients personal care activities
  • Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
  • Maintain patients care records and document provided services
  • Assist patients with mobility and physical therapies/exercises
  • Instruct and counsel patients and families on diet and exercise
  • Collect routine specimens
  • Provide companionship and basic emotional or psychological support

Required Skills/Abilities:

  • Working Knowledge of community resources
  • Ability to monitor vital signs and to collect specimens
  • Familiarity with basic nutrition and personal hygiene standards
  • Caring and compassionate personality
  • Good interpersonal skills
  • Proof of current CPR, TB screening, and Hepatitis profile

  • None

Required Education and Experience:

  • High school diploma or G.E.D. certificate 
  • Certified Nursing Assistant (CAN) or Home Health Aide (HHA) certificate
  • Current Texas Drivers License

Physical Requirements

  • Prolonged standing, walking and sitting required
  • Requires working under some stressful conditions to meet deadline and patient/family needs
  • Make quick decisions and resource acquisition
  • Meet patient/ family psychosocial needs

Work Environment:

  • BLOODBORNE PATHOGENS EXPOSURE LEVEL: 1 (HIGH)
  • Noise Level: Moderate
  • Works in patient homes in various conditions
  • Ability to work flexible schedule and ability to travel locally
  • Transportation: Reliable transportation and valid and current auto liability insurance
